Medical coverage is provided through Gravie, using the Cigna OAP National Network. 

Employees may choose between two medical plan options:

• Gravie Comfort Plan – $5,000

• Gravie Plan – $3,000

Both plans use the same nationwide Cigna network. The difference is how costs are shared through deductibles, copays, and out-of-pocket maximums.

Who is Gravie?
We process and pay your medical claims. Gravie Administrative Services, LLC is a licensed Third Party Administrator (TPA) that manages self-funded health plans for employers across the U.S. Providers should call Gravie Provider Services to verify eligibility or coverage details for specific procedures at 833-486-3239.

Preferred Provider Organization (PPO):
A PPO allows you to see any provider when you need care. When you see in-network providers for care, you will pay less and get the highest level of benefits. You will pay more for care if you use non-network providers. When you see in-network providers, your office visits, urgent care, and prescription drugs are covered with a copay and most other network services are covered at the deductible and coinsurance level.

HOW CAN I FIND OUT IF MY DOCTOR IS IN-NETWORK?

• If your doctor accepts Cigna OAP National Network, they are in-network

• View in-network providers by logging into your Gravie account.
